To RCTC Director of Admissions:
I am petitioning for a withdraw from Fundamentals of Public Speaking 1114 and Golf Team 1236 for medical reasons. I should inform you that in the spring of 2007 I was diagnosed with depression and in August 2009, had a Pilonidal Cyst removed from my tailbone (see attached medical documents and hospital records). First, I have been hospitalized 4 times for depression and have been seeing a psychiatrist for 2 years now. Depression casues me to have anxiety attacks and to feel unconfortable about myself. In my speech class, I found that the anixety was far worse than I thought it was going to be, and it’s unheathly for me to feel this way. Second, I had a pilonidal cyst removed from my tailbone. The surgery was peformed 3 weeks before classes began at RCTC and had the stiches removed 10 days before golf practie started. All was going well until the second week of practice. My incision started to drain blood and became infected. The pain of swinging the golf clubs and carrying a bag 18 holes was to much for me. I informed Coach Brain LaPlante of my injury, since then I’ve had another surgury to remove the infection. I have the incision packed 2 twice a day, it’s difficult for me to sit for a long period of time, and I cannot do any extreme movements (swing golf clubs, ride a bike, etc.). Since I was unable to fully participate in Golf Team 1236 , mental health issues interfear with my ablity to feel confortable in Fundamentals of Public Speaking 1114 learning invironment, I am being seletive in asking for a withdraw for these two courses.
This letter serves as my formal petition for medical withdraws from the two courses and my explanation for both circumstances.
